- Level 2.1 Intensive Outpatient (IOP) — structured programming 9–20 hours per week
- Level 2.5 Partial Hospitalization (PHP) — intensive daily treatment 20+ hours per week
Coverage typically includes clinical assessments, individual therapy, group therapy, case management, and medication-assisted treatment coordination.
Am I Eligible?
